I have a 2.1 rack, is a 2.5 one actually different, or just the track rods?

Track rods and track rod ends are larger but interchangable.
Only restriction is each rod and end must be the same.

How long has this been going on? Did the symptoms appear all of a sudden, or was it something that started small and then gradually became worse? Has any work been carried out on the hydraulic system?

Air in the hydraulic fluid can produce stiff and notchy steering, and lack of smooth power assistance.
Might be worth checking this out.
After driving the car for a short while (or running the engine and moving the steering wheel) dip a sample of LHM from the reservoir with a small clear container. Fluid should not contain bubbles, even microscopic ones.

Yes, that's something I have experienced and had in mind when asking my questions.
